ม.มหิดล เปิดตรวจสอบสิทธิ ยืนยันตัวตนฉีดวัคซีนโควิด

มหาวิทยาลัยมหิดล เปิดระบบให้นักศึกษา และบุคลากรตรวจสอบ และยืนยันสิทธิ์การเข้ารับวัคซีน COVID-19 ณ ศูนย์ประชุมมหิดลสิทธาคาร ศาลายาของนักศึกษา และบุคลากรมหาวิทยาลัยมหิดล รอบที่ 1(เฉพาะผู้ที่ลงทะเบียนผ่านแบบสำรวจการรับวัคซีน COVID-19 ของศูนย์การแพทย์กาญจนาภิเษก ระหว่างวันที่ 6-20 พฤษภาคม 2564 และกรอกข้อมูลครบถ้วน เท่านั้น)เข้าสู่ระบบ และ "ยืนยันสิทธิ์" การรับวัคซีนตั้งแต่วันที่ 28 พฤษภาคม – 1 มิถุนายน 2564 ที่ https://vaccine-regist.mahidol.ac.th/หรือ SCAN QR CODE

** ปิดระบบ เมื่อมีผู้ยืนยันครบจำนวน (จัดสรรตามลำดับการยืนยันสิทธิ์)

อย่าลืม!!  Capture screen หรือ print out หน้าจอ “ยืนยัน” การรับวัคซีนDownload เอกสารประกอบการฉีดวัคซีน และกรอกเอกสาร ส่วนที่ 1 ข้อมูลสุขภาพ และส่วนที่ 2 แบบประเมินความเสี่ยงนำติดตัวมาในวันมารับวัคซีน

for Staff Members and Students Round 1  (Reserved for staff and students who registered correctly and completely through Golden Jubilee Medical Center COVID-19 vaccination survey during May 6-20, 2021)
